A family history of hypertension is associated to
nephrosclerosis and predicts the future onset of hypertension
in patients with IgA nephropathy

This study examined the progression of IgA nephropathy in 2
groups of 29 patients matched for age and sex. However one of
the groups had a family history of high blood pressure, and one
did not. Mean age was 36 years in each group and mean duration
of follow-up was 4.6 years. At the time of initial renal biopsy
96% of IgA patients with (+) family history of high blood
pressure had evidence of nephrosclerosis, vs. only 10% with a (-)
family history. At the end of the follow-up period, 90% with (+)
FH had hypertension vs. only 10% with (-) FH. The prevalence of
renal failure in patients with (+) FH was 45% vs. only 3% in
patients with a (-) FH. The results suggest that nephrosclerosis
in patients with IgA nephropathy has a strong genetic component
and is linked to a family history of high blood pressure. The
latter is an unfavorable prognostic indicator of progression in
this disease.
