Progression of diabetic nephropathy and the insertion/deletion
polymorphismof the ACE gene

In this paper by Tarnow et al. 40 diabetic patients with insulin-
dependent diabetes mellitus were studied. They all were
normotensive. Rate of progression over a 3 year period was
measured and correlated with presence of the DD ACE genotype. 19
of the patients had the DD genotype. In this group there was no
difference in the rate of progression from that in the group with
other ACE genotypes, although there was a trend in the DD group
for a faster rate of decline in ClCr (3 ml/min/yr vs. 1.1
ml/min/yr in non-DD patients).
Comment: Although
this study was negative, the trends were such that they suggest
that DD ACE genotype diabetics may indeed suffer from accelerated
decline in renal function, and larger trials may reveal a
positive result.
