Glomerular number in focal segmental glomerulosclerosis and minimal change nephrotic syndrome

This study examined the hypothesis that progression of minimal change nephropathy (MCN) to focal glomerulosclerosis (FSGS), or that the partition between these two pathologic findings may be related to the number of glomeruli. Kidney biopsies from 32 patients were analyzed. 10 had MCN for more than 15 years with no progression. 11 had steroid-resistant FSGS. The remainder were age-matched biopsies from renal donors. The glomerular volume was estimated from the kidney volume as determined by IVP or renal ultrasound. The results showed that the estimated number of glomeruli was similar in patients with MCN and controls, but was substantially reduced in patients who had FSGS. The result lead to a hypothesis that patients with a low number of glomeruli may be more susceptible to develop FSGS. (Daugirdas)
