Renal lesions in drug induced lupus syndromes

Renal involvement in drug-induced lupus (DIL) is rare. 18 patients treated with hydralazine, procainamide, phenytoin, propylthiouracil, or other drugs were studied. Of 16 biopsies, 5 showed proliferative GN (2/5 anti-DNA +), 6 showed necrotizing GN (4/6 ANCA +); other lesions were mesangial GN, membranous GN, minimal change, and renal vasculitis. In conclusion, both immune complex and pauci-immune GN can occur in DIL. Therapy includes drug discontinuation plus immunosuppressives. (Leehey)
