Comparison of 3 iron dextran infusion methods to correct anemia
in erythropoietin treated hemodialysis patients

Intravenous iron dextran is usually administered in an empiric regimen of
100
mg every hemodialysis for 10 treatments. This group studied 3 dosing
protocols in iron deficient HD patients: The standard regimen, 500 mg in one
weekly dose, and a total dose infusion in which the entire iron deficit was
repleted in one infusion. The investigators found no difference in efficacy
or occurrence of side effects between the 3 regimens.
Comment: The total dose infusion is simple to administer, is
convenient, and
probably cost effective. I am concerned, however, by previous studies
showing
an increased risk of mild side effects (arthralgias and myalgias).
